All paperwork is completed annually per NCAA requirements. If your paperwork is not submitted in full by your deadline, you will not be permitted to participate. A parent or guardian must sign all forms if the student-athlete is under 18.

Medicare, Medicaid, Tri-care, and Christian Ministries plans are generally not accepted as primary insurance. If you do not have eligible coverage, talk to your athletic trainer about these options.
